PRODUCERS CERTIFICATE
I, YOUR FRIENDLY FARMER, market my milk through MY LOCAL COOPERATIVE Producer # 123456 and am responsible for the milk production marketed under this producer number. I hearby certify that NO MILK COWS will be treated with rBGH or rBST which produce milk to be marketed under the MY LOCAL COOPERATIVE Producer # 123456 without thirty days prior written notice to MY LOCAL COOPERATIVE .
I the undersigned producer, hereby expressly authorize MY LOCAL COOPERATIVE to use this certificate in the marketing of milk marketed under the MY LOCAL COOPERATIVE producer number listed above and recognize that one or more third-parties will be relying on these representations.
I the undersign Producer acknowledge:
That MY LOCAL COOPERATIVE may pay a premium because the milk from the dairy herd of the undersigned producer is not treated with rBGH or rBST;
That the level of premium is subject to change
That the premium may be eliminated
That the obligations and representations of the undersigned Producer contained in this CERTIFICATE shall continue in force and effect without regard to the amount or existence of any premiums.
I, the undersigned Producer, hereby authorize MY LOCAL COOPERATIVE to make such disclosure to any government or regulatory body of the existence and contents of this certificate as may be required by law or administrative regulation.
I, the undersigned Producer hereby acknowledge and agree to accept all liability to the parties relying on this CERTIFICATE who may incur monetary damages resulting from misstatement in this CERTIFICATE and/or any breach of the obligations in this CERTIFICATE .
I have read the foregoing CERTIFICATE , know the contents, and hereby certify that the matters and things alleged in the foregoing CERTIFICATE are true and correct.
